You should verify your google map api key quota -

The error is "You have exceeded your request quota for this API" take a look at the page explaining this .

I still can see the same error , maybe you can try to use our newly released plugin ja open street map and avoid google api key , take a look HERE

I cannot be of more help since it is a pretty google map api configuration.

To download it you will need of a JAEC Membership , that allows to download all joomlart commercial extensions not only ja open street map .

Actually your membership type allows to download only joomlart templates and complimentary extensions.
