Captcha on the last line

can you help me make the captcha on the last line or make the extra field that's right after on the front line ! the simplest to do !

something is missing ! I added the code in the folder script.js but it did not work! I created a custom.js file which I inserted the code it did not work too! I was wrong in something ?

Thanks saguaros for your answer, but he has profile fields instead of custom fields for me. and there I ignore the path of the file that contains the code of the custom field.
