No, it's not index file of your root folder, it's from T4 core file:
/plugins/system/t4/themes/base/html/layouts/t4/index.php
We overrode and modified it in template for you. The same with the css, we put in custom.css file so it will not be affected when you update template or T4, I mentioned in my previous post.
