Calendar icon and date select missing

I am using JADonate and Joomla 3.9.14

I have a membership form in Civicrm with date fields. However, in the frontend, the calendar icon where you can select the year, month and date is missing.

I would appreciate your help to make this feature work.

and there are 2 fields with date inputs: date of birth and wedding anniversary.

Also please let me know how to send to you the credentials.

I tried to access site but its same, it appears my IP is blocked by your server.
I tried USA VPN for access the site and its giving 404 error on site : http://prntscr.com/qhcg3w
Kindly check the access issue.

You can try this suggestion : Open JA Donate template style > Themes Tab > Legacy compatibility > Turn ON.
save changes , clear cache and check.

maybe its issue of IP
My team member will look if he can see the site, Please check the membership page with default Joomla template Protostar and see the icons showing or not.
If its not showing with default Joomla template in this case the problem is not template as the same output is by JA Donate.

SInce its not showing with default Joomla template, there would no such option, i suggest you to contact the extension provider about its icon features.
If the icon show with default Joomla template it will also show with our template.
