where is the 'extra fields' tab

I am creating a restaurant page in Uber. Using the ACM I tried to set up the 'Food in the News' in the Articles Category Module. On the 'building uber sample sites' how to page it shows an 'extra fields' tab showing in the module but I see nothing on my admin end. There is a 'module options' tab but it does not show the same parameters that the 'extra fields' tab shows. How do I get the 'extra fields' tab to show in the 'articles category' module. Thanks.

OK. So it looks like the 'module options' tab is now the 'ja extra fields' tab? However the 'sub heading' text and the 'background image' are not showing in the front end.

In order to show that 'JA Extra Fields' tab in the module, you need to set Uber as default template first.

Then you can add module text like below:
