cspgsl The conflict is from the RSform 2, Please compare it with your form 1 settings that is on Appointment page, it only make issue when your use RSform 2 in article. Apply the same settings and check.
I could see that it might be an issue however, the forms and articles are exactly the same as in this site.

adding the coordinates broke the map on this page so I removed them https://canadianaccountantsforum.ca/contact/appt
I ran my mouse over the page with the inspector window open and see that there is a difference between the two page. this screenshot is of the two positions I see when hovering on this page https://canadianaccountantsforum.ca/contact/email
