Why {target} is not replacing the actual value in activity

There is wrong translation in your language package these strings that is not picking correct value.
Kindly check the translation file and find the string, compare its value with EN language file.
You can only change the words do not change any syntax in the language values.

I don't think it's a language file problem.same things are showing in English language also.
It's a issue of PHP+Jomsocial. because it replace the {target} & {actor} with user name.
Please make sure and let me know

After checking your site i noticed these item that has issue are not from JomSocial core. Your site has a 3rd party plugin JReviews - JomSocial Activity Stream that used to add them in JS wall stream. kindly report the issue to plugin provider as its from the 3rd party plugin.
If there is error in the JomSocial to add photo, positing content that is by JomSocial side let me know.

I have un-installed the JReviews Jomsocial Activity plugin but still getting the error.

These activities are already added by the plugin in site DB. To remove them you need to delete them manually.

I have migrated data from J2.5 to J3.XX. and old site is working fine while i copied these data from this site.

Its not related to Joomla, JomSocial does not belong to 3rd party settings. If the plugin is not supported for new version of JomSocial it will not work you need to check with plugin provider as this is not issue in JomSocial.
