conflict with js.html5lightbox

I have a problem with js.html5 lightbox since if I have it active, my contact form does not work and if I deactivate it my form works. My question is how can I make the plugin activated on the pages where the video will be shown? and disable it on the pages I don't need it.

Is this form from 3rd party extension? as it doesn't look like our demo site.

You can share the credentials of your site eg: URL + admin account and tell me that page so I can help.
