Add fields to the contact form

how can I add some extra fields like adress, ... to this form?

It belongs to JA Quick Contact module, you will have to customize it a bit via this file: /templates/ja_landscape/html/mod_jaquickcontact/ajax_layout.php

to add new field.

Here is my tweak to add an address field:
