Contact form is going up , over the menu

I think after upgrading T4 plugin my contact form is over my menu now, it's going up

Can anyone help me what to do so the contact form is on his place ?

Before conti. support on this topic, i have not found an active membership tag on your profile for JA Landscape template support. As per support policy only active members get support and updates for Pro templates.
