"Show Module Titles" when set within a module itself are not displaying on your b5 t4 blank template.

It may due to the module style you're using which doesn't render the module title.
You can share me the the login info of your site and the module you're referring to, I will check for you.
I confirm this bug report. I spent an hour last weekend trying to get the title to show. In the end, I gave up.
I tried all the module styles in the attached image and the title would still not show.
@pstanbra could you share the credentials of your site eg: URL + super admin account and the module name so I can check?

The module name is called "Featured Businesses" it is located on the home page of my website.
I am not using t4 page builder. Just the b5 template with plain custom modules.

It seems that you did not follow my suggestion, I just changed the module style and it shows the title normally:
