Contact Form is Blank

On the quick install version of Educasty the contact is setup proper, but it loads nothing but the title "Contact" on the template. None of the details show. I see someone else had a similar problem. Would I be able to get the proper PHP file code for this page? (or whatever fix) so I, and others, can just copy it?

I installed a new template. All I did was change the menu title from "Contact Us" to "Summon Us" and left the alias to contact-us and it breaks.

If I change the title back it stays broken. So simply changing the menu name (not alias) breaks the contact form.

Not sure how to fix. When I re-establish and choose (single contact and select name) it stays just like the picture in post 1.

I have just edited the original post to place credentials. If you can inform me of how its fixed so i can apply it to my other 3 sites with this template. Thank you!

Thank you! I'm going to save it. I copied the default.php contact file from ja_alpha into my localhost server and that worked too.
