Code from default.php appears below the module

Hello,

Concerned site / issue is visible at : https://unat-idf.fr/annuaire
On the right, you can see the JAK2 filter/search module published with the title "Rechercher dans l'annuaire".
Just below button "rechercher", you should also see some code beginning by "'; if (jQuery.inArray(jQuery(this).val(), ['adate']) === -1) strout += '
'+ '" ...

The button is also inactive when clicking.

I've just upgraded the module to 1.3.3 release.

Please help.

As a temporary solution, I managed to removed the displayed code by removing several lines of code within : /tmpl/default.php as mentioned on attached picture.
But it will be great if I can get a clear explaination about what's going wrong with those removed lines.
