It's page heading and I disabled it in menu item: Driver Application

About the image on top, you can update via JA Masthead module, I just added a new condition:

[Masshead Itemid="137" title="" background="images/joomlart/masthead/bg-1.jpg"]FORM TO APPLY FOR DRIVER[/Masshead]

You can change background image by your desired image.
