May I know which template are you using?
By default, T4 Blank doesn't have this extra fields tab.
Hi Saguaros, thank you for your reply. I really appreciate your help. I am using ja_stark - Default. The default is none or 2 columns for Module Layout. I wanted to have 3 instead. Is there a ways for me to increase it? Thank you.
It's custom layout form template provides for the JA ACM module, I'm afraid that you must customize it on your own 🙂

You will customize several files:
/templates/ja_stark/etc/form/mod_ja_acm.xml /templates/ja_stark/html/layouts/chromes/T4Section.php
and style also.
