Contact form not displaying correctly

Hi,
My contact form is displaying as the sliders but I just want it to show the standard contact form and I cannot find the menu item that allows you to change this.
Also on the sliders there is a 'Get in Touch' message with what looks like a broken image link next to it.
Ive attached it as an image.

Please can you take a look?

Hi, apologies, I managed to fix this with your updated default.php file I found from a previous customers same issue.
