It looks like you're using old version of T4 plugin, here is how it should look with latest version:
You can consider to update latest version into your site (Remember to backup your site first)

I have just realized that firefox displays corrupt but chrome not. My screenshots are firefox's screenshots.
Could you try to open with private window of Firefox?
