Template cache issue randomly with wrong presentation

hi saguaros, cannot update the case as its closed, mantha just goes on the website for a sql query, and we face this issue with template (that what not the case before but the cache was disabled)

time to time (randomly) ja good is not displaying the article correctly (cleaning the cache fix the issue) and randomly the bug came back, i've tried to disable some plug in like, vote, no luck
