Best way to display a contact form

Any suggestions what the best method would be to show a contact form in a module so that it looks something like this:

I've actually tried JB contact - installed now in Grid 13 position on the site. Are there any ways the improvement of this module can be improved? eg Change the text 'Send an email', remove border and make background transparent?

Try add this to custom.css file :

.bfQuickMode .bfLabelLeft input, .bfQuickMode .bfLabelLeft textarea{background-color:transparent !important;border: 0 none !important;}

Kindly ensure to clear your browser cache if changes dont reflect

Quick contact modules are a trade off between complexity / functionality and ease of use compared to other more complex form extensions
