I have inadvertantly deleted the contact form that comes with Application. Under instructions to clear cache regarding another problem I rendered it unretrievable. I looked for the information to recreate it and am unable to locate it. Where is this documentation located? Does it exist?

Below are details of contact form :
In the top there is Masthhead module . you need to configure it for the menu .
this doc will help you : http://www.joomlart.com/documentation/joomla-templates/uber-template/extensions-detail-instruction#ja-masshead
In contact form , create menu type single contact > select the contact .
contact > component > contact > create a contact to use it in above menu
display format is : Plain in contact option > http://prntscr.com/89wsdc

in the lower part of the page u can see there is google map module that is loaded by the JA ACM tabs container .

I believe the instructions are out of order and i should create a contact fist in the “Components” menu.
Should I assign the menu item I created to the Contact Page I created?

I’m having some difficulty understanding your instructions without visual cues to guide me.

pankajsharma — I found some guidance searching the forums. The final thing that I had to figure out was using a layout with ‘component’ in it. I was using Home which does NOT include ‘component’. The appearance is still not what I want it to be, but I’m closer to solving the issue.

Attached is an image of what this configuration is producing:
