Hi
Its default Joomla contact forum .
You need to create a Contact item from Joomla contact component and then create a single contact Menu and select this item .
Menu settings : http://prntscr.com/avopdc
Contact item : http://prntscr.com/avopjy
http://prntscr.com/avopms
