When I add a Contact Us to the main menu, (just a single contact) it will not show the contact form. it shows the words “Contact Form” but nothings there, and nothing happens when i click on it.

the Contact Us i added on the top menu at the very top of the page works fine!

please help!

http://184.173.196.222/~ctf20/ here is the site. the contact form works from the one on the top menu, but not on the one on the main menu. thanks for your help.
