How i can have a Contact form page similar to template Ja_Muzic…

The contact form files themselves should be within the following file paths (respectively):

Contact Form Template – templates/(template name)/html/com_contact/contact/

Address Fields – components/com_contact/views/contact/tmpl/

From there, what you would need to do is download the template files for JA Muzic and try to duplicate the settings and styling for the JA Muzic contact form into your JA Mitius based site.
