Can’t seem to figure out if it’s a preset module. Would love to incorporate something like that into my site without having to recreate. Any help in pointing me in the right direction would be awesome.

I’m assuming you installed the quickstart + sample data package when you installed your Purity III template, yes?

I originally only installed the template and have since installed the quickstart but I’m not sure if I have what I need to duplicate the contacts module I was referring to.

Within which Purity III layout is that contact module display?

You can use this contact page by configuring as below:
1. Config the Contact Us menu : http://prntscr.com/4j6lsd
2. Create a custom HTML module : http://prntscr.com/4j6m42
a. Assign this module to sidebar-2 position
b. Assign this module to Contact us Menu
3. Assign the Login form:
a. Assign this module to sidebar-2 position
b. Assign this module to Contact us Menu

4. Assign the Menu Contact us to Purity Default Style (With the layout: Default-Content-Left)
