Could u provide more details with site url .
the one page work only when use the settings as shown in the documentation , If template is not JA Onepage and using a different them i am afraid it will not work .

Hi Kindly check the page http://executiveassistant.com.ng/index.php/sub does this page have your form module ? I do not see any form published on this page . Make sure the Menu item sub use a template style that has component section in its layout .Use two template style if you have one template style , that can be issue . Assigne the new template style to SUB menu and use default layout in it . I hope it works .

Hi
The Menu Item SUB is using template style JA METRO that does not have component block in it .
I assigned default template to it and now its working .
Beside the page is showing blank because there is nothing to show by the Menu type (chronoform). You can set the template style to Default Joomla template and see the result will be same .
You need to check the 3rd party extension configuration and contact to their support team for its working issues .
