How can I line up these form boxes so they are lined up correctly?

You try to do following way below :
– Download attachment file and extract it
– Copy and overwrite to componentscom_userviewsusertmpl folder .

Would I be able to use this form fixed file on all templates with JA? I also have a site with AVIAN II and the details screen has the same issue.

Yes , this is problem in code of joomla , so you can use it in all joomla site with same version. good luck !
