Hi
You can Google map module on a module position on the page that using Breez. Form.
This way you can get both on the same page.

I don`t know how create a module with Google map.

Is there some special way to do it?

Hi
Create a Custom html module from Module manager.
Add the Google map code in the Module editor.
Assign the module to the menu items.
save module and check.
