Hello,

I would like to create a contact form that has fields similar to the Article Make An Appointment but which has the “send email” button which the make-a-appointment contact form contains. How can I do this? Is there any documentation I can refer to?

The contact form Make-a-appointment in our demo is a static content ( a article) and it does not work like a dynamic form. If you want to make similar form, you have to use 3rd party form extension from JED

In our demo we do use custom HTML to illustrate various aspect of the concept.

What about hte contact form itself? Where can we add/remove fields and change the “Miscellaneous info” heading?
