Hi,

I looked into it and I followed and it appears well functioned like shown in the first attachment.
However, when I send a test e mail, after having filled all the fields, I get a message shown in the second attachment.

Where could I solve this?

This is strange if the custom work , working with default joomla templates , it should work with JA Sugite , i need to check this bug directly on your site , kindly send me site admin URL with super user login details .
