You can manually add custom HTML code and link to your logo file here. If you can provide where’s your logo file located, I can provide you the code to add here.

Thanks for the info. You can navigate to the module setting >>> switch to tab JA Extra Fields >>> under Intro Text >>> add this above the current text :
