Hello
i am unable to access your site > http://i.imgur.com/Fw5QBgH.png
Make sure you are running on Latest T3 plugin version

Before working with Custom HTML module, please disable the default Editor under Global Configuration .
Editor remove the icon code so set editor to None and try to add the code again as mentioned in documentation here

due to dns issue on your site > http://prntscr.com/7taei0
Meanwhile Did you added the module source code again after set the editor To None ?
I can check it once the DNS issue is resolved on your site.
