How can I setup my homepage exactly like the demo site is setup, with the slideshow placement and the banner placed on the left side? I cannot find where the middle.content.mass-top is available for selection for the slideshow.

JA Bistro template is just supported to user has license of joomlart. So you buy license to be supported by us, please.
