Hi

There maybe conflict in Bootstrap used in JA Healthcare, you can try this tweak:

• Open the file: ROOT/templates/ja_healthcare (your default template)/css/custom.css (create this file if it doesn’t exist)

• Add this css rule:

  .modal-open .pwebcontact-modal.modal.fade.hide.in {
  display: block !important;
  }
