Hi, is there a way to create a membership form featuring the specific fields I want (i.e. Name, Institution, Specialism etc etc)? What I want is for visitors to fill in a form and then proceed straight to PayPal payment.

I have searched extensions but all I can find is the contact form.

For this I would suggest one of the third party components such as RFforms. With such extensions you can create any form you wish.
