I’m using the Perfect Web Ajax Popup Contact Form on my site. I have in the “Left” module postion and it displays correctly. It also displays on the Right Menu position with the title of the module. Images of the website page and the module settings are attached.

This is problem your mention related to a 3rd party extension (Perfect Ajax Popup Contact Form). You should also raise this in the perfect-web forum, you might get better answers there.
