Hi,
The template (JA_Mono) did not display properly with the settings JS & CSS optimize set to "on". All formatting was lost. Do I need to exclude any JS script?

Also, after I reset the optimize option to "off", I have lost the layout options (see attached). Could you help to get the setting choices back (ex number of columns in footer, responsive settings etc.)

The issue is from the CDN as you can see Here http://prntscr.com/ejn5rl
Cloudflare is not delivering the files this is reason you are not able to see the layout. I can still help you to make the footer in 4 columns but i suggest you to post issue on your CDN for this or try to disable it permanently and see it work.
I tested the package of this template and i have no issue on my localhost.

Kindly Increase the Memory limits and max execution time from server PHP configurations.
The seems low and its unable to load the layouts.
Also Joomla not showing the PHP configurations .
