Hmm, have you got any link to live site, pls provide me and the administrator account, i will check this issue in detail.

And I could create a poll in this forum in 4 steps without any problem:

1) Click to the new poll button as you mentioned
2) Select the Select a Forum
3) Select a Type In the Create A new Poll Select box
4) Create a list of poll item.

=> Save > done

Please try as above.
