The left side title is the module title of contact acm module Here >> http://prntscr.com/b8ia0x
Change the contact module title with your title .

And if you want to change the fields name in contact form
Go to /languages/en-GB/en-GB.mod_jaquickcontact.ini
file find and change them >>>> http://prntscr.com/b8i7cs
