How do I get the JA Barite sliding module that’s in the demo in the slider module position?

and where are the module suffix paremeters for this template?

I think this is JA Content Scroll module.
It could be downloaded from the JA Barite download page.
Also, if you download the quick-install package from there, you’ll be able to check all configurations/settings of the demo site.
