How does one achieve the 3 dimensional box effect on the user 6-10 positions?
on my site http://www.coleswoodwinds.com. the modules are just plain

I installed the quick start on my local machine and it has the boxes etc, yet when I look at the modules they are not using any special suffixes.

You can create in custom modules to have same appearences as seen in demo template and then enter this in custom output of specific module::
